Human Performance Engineering Pty Ltd was established in 2006 to bring to the workplace, a variety of practises from the Nuclear Power Industry.

 The Chernobyl accident on 26^th April 1986 
 was due to various causal factors.

 One major contributor was their lack of 
 ‘safety culture’. This impacted on their

 ability to challenge design weaknesses 
 (a  latent error), even though it was

 known about prior to the accident. The 
 error prevention barriers put in place

 through procedural compliance and 
 adherence were broken.

 Confirmed communications was another.
